Application for Service. It shall be the responsibility of the customer to apply for utility service or discontinuance of service by signing a service order at the City Administration Building, Kenai, Alaska. 2. Payments is Lieu of Assessments. -6- f. _ _ `~ ~~ (a) The amount ~f payments in lieu of assessment for areas inside the City limits shall be determined on the basis of the total post involved, plus the portion of the cost attributable to the central system, plus such additional percentage ,3s may be determined to be reasonable. These payments are payable in five annual installments including 6`/o interest on unpaid balance if the property owner chooses to sign a r~ortage to secure payment to the City. The first payment is due 30 calendar days after placing an application for water service, and the other f9ur are due in four annual installments. (b) In all ca;;es when additional area is utilized and/ or expansion of the system occurs after the initial payment in lieu of assessments, an additional payment in lieu of assessment will be required in a•~cordance ~lith the procedure outlined above. This will be computed on the basis of the actual proper.•ty to bE served that is in ex~:ess of the area for -~hich the initial in- stallation was made. (c) In areas within the City limits wY:ere city mains do not exist and construction is desired by abutting property owners, the property owners desiring the service (if they repre- sent owners of 50'~ or more of the property benefited) may petition the City Council to form an improvement district Yor installation of said service. The total construction costs shall then be assessed to the benefited properties for payment of said installa- tion. 3. 4later Connections and Extensions: (a) The plater Utility Division shall install and/or replace all vrater service connections from the main to the prop- erty line. This will include the keybox and thaw wire. Eaccava- tion and backfill for this portion of the connection (up to the property line) will be performed by a contractor under the di- rection of the Water Utility Division. The charges for this work shall be payable at the time application is filed for the service connection except for the initial 100 connections to be installed in 4! & S Improvement District ~}1 provided that the first year's installment shall be paid on application. All con- nections, and the materials used, shall be subject to the inspoc- tion and approval of the plumbing inspector prior to backfill. The charges for the various sizes of service connections and the total connection charge shall be published annually by the Water Utility Division. A minimum deposit of $300.00 will be required where paving is cut, in addition to the published charge for the service connection, except where the pavement c~~t will be longer than ~b the width of the pavement, in which case a higher deposit, as determined by the Department, shall be required. (c) All plumbing, including the extension of the ser- vice connection, shall conform strictly to the Plumbing Code, Sections 4-20 through 4-25, City of Kenai Code, 1963, and shall be subject to the inspection and approval of the plumbing inspector. (d) The plumbing inspector and other authorized city personnel shall have free access, at reascr~able hours, to the premises and buildings where water service is provided for pur- pose of inspection and in conformity to these regulations. Ser- vice may be discontinued should the owner restrain authorized personnel from making inspections, or should the owner fail to repair any leaking extensions or plumbin6. (e) All turn-ons and turn-offs and the operation of the keybox at the property line shall be made by City personnel only. A fee of $20.00 will be levied on requests for turn-on and turn-off during periods other than normal duty hours, and whenever such action is required due to delinquent account. (f) All customers shall be required to provide a separate stop and waste valve whero they do not exist inside the building being served. The valve shall be located on the service entering the building ahead of any branch lines where it is readily accessible in event of emergency. (g) Each and every building served by the Water Utility shall have a separate outside shut-off. In the event it is impossible to get a separate outside shut-off within the public right-of-way, the proper~y owner shall provide the shut-off within private property and execute, in favor of the City, an easement providing access for the City personnel to the keybox. (h) The property owner shall be responsible for notifying the City, and shall sign a disconnect request if a ser- vice is to be abandoned. All abandoned services shall be dis- connected at the main by City personnel. (c) Disconnections of rater service must be made on the building side of the service box, or it must be ascertained that no grounding wires or cross connections to the electrical system exist before thawing electrical?y. (d) It shall be the property owner's responsibility to know the location of, and have marked, the keybox and thaw wire on the service connection. 7.
