Date: la I )09 (if applicable) City of Kenai Mini -Grant Application The Kenai Peninsula Wolf Club, under Bridges Community Resources Network a 501 c 3, nonprofit organization, would like to request support from the city of Kenai. The Kenai Peninsula Wolf Club is an after school program for youth in 1 to 12 grade and currently we have 27 youth in our programs. The mission of the Kenai Peninsula Wolf Club is; To foster the well being of our youth though safe, healthy, interactive activities to enhance skills that will be needed in the future. To provide youth with the opportunity to excel in creative, unique ways, while instilling cultural awareness, and understanding of each other. The Kenai Peninsula Wolf Club, (KPWC), will offer interactive, healthy activities for youth who reside on the Kenai Peninsula. Currently KPWC has a Junior and Senior Native Youth Olympic (NYO) teams, with a group of athletes that competes every year at the World Eskimo Indian Olympics in Fairbanks. KPWC also offers inner tribal drumming, beading, and help with academics. KPWC has three volunteer staff that act as coaches, tutors, mentors, and provide positive role models for these youth KPWC offers after school activities three days at Kenai Middle School from 2:30 to 6:00 pm. The KPWC programs help strengthen youth and families, which in turn, strengthens our community. KPWC measures the success of the programs through surveys to youth and families and through the academic progress of our youth. All youth in our programs must maintain passing grades throughout the school year. KPWC youth program started in September 2009 and will end in May of 2010. We would like to request support of the Kenai Peninsula Wolf Club after school activities in the amount of $500.00 which will help provide equipment, craft supplies, and more importantly healthy after school snacks for these youth. Oversee the daily operation of its programs, d. Select and direct the Miss Dee Dee's Class staff and volunteers working on its program, e. Set goals and objectives for contract employees, and negotiate and approve their contracts, f. Create, approve and follow its budget in compliance with the requirements of its funding source, g. Obtain, pay for and monitor its own telephone for making long distance calls and an answering machine for receiving confidential messages, h. Provide to Bridges copies of all required documentation, including but not limited to grant proposals, by -laws, personnel agreements, and personnel and program policies, L Miss Dee Dee's Class staff and volunteers will respect the right of other Bridges' users to their own opinions and beliefs. Through and by its staff, assist per grant details, administer as specified in the grant, and compile necessary financial reports on a mutually agreed schedule, c. Provide accounting services as approved by the Bridges board of directors, including banking services regarding funds received, bookkeeping, payment of outstanding bills, preparation of payroll, and preparation of and submission of the appropriate forms for any employment taxes due, such taxes to be paid from each employers fund, d. Maintain and pay for a Post Office Box at which mail can be received, e. Take messages received at Bridges' phone number and forward as needed, f. Maintain a nonprofit mailing permit, which can be used for mailings, by Bridges, of materials which conform to Post Office regulations, g. Bridges agrees to provide access to miscellaneous services, including but not limited to: Borough sales tax exemption cards, use of the Bridges' credit card machine for a fee, use of the Bridges' State of Alaska gaming permit for raffles, and use of the photocopier, with the costs and /or fees associated with any such services actually used to be reimbursed by user, h. Bridges agrees to maintain and make available upon request all book, records, documents and other evidence pertaining to costs and expenses relating to this agreement to the extent and in such detail as will properly reflect all direct costs of labor, materials, equipment, supplies, services and other costs of whatever nature for which reimbursement is claimed or payment is made under these provisions. 5. This agreement shall be subject to all applicable provisions of State and Federal law and regulations related to the delivery and funding of nonprofit social services. SERVICE FEES AND EXPENSES 6. Miss Dee Dee's Class shall reimburse Bridges a 5% grant Administrative fee from each grant for which Bridges is responsible to be administrator, and agrees to reimburse any indirect or additional direct expenses incurred by Bridges and approved by its board. Pay the Bridges $25 yearly membership fee. CONFIDENTIALITY 7. Both parties agree that they shall be bound by and shall abide by all applicable Federal or State statutes or regulations pertaining to the confidentiality of client records or information, including volunteers. The parties shall not use or disclose any information about a recipient of the services provided under this Agreement for any purpose not connected with the parties' contract responsibility, except with the written consent of such recipient, recipient's attorney, or the parent or guardian of recipient's who have a parent or guardian. LIABILITY AND INSURANCE 8. Bridges shall obtain general liability coverage for Bridges' prem plus Director and Officer Insurance. 9. Miss Dee Dee's Class is solely responsible for liabilities arising out of its program and its interaction with participants in its program. Miss Dee Dee's Class specifically indemnifies Bridges against any and all claims arising from the actions of the governing body of or paid and volunteer staff of Miss Dee Dee's Class. Set goals and objectives for contract employees, and negotiate and approve their contracts, f. Create, approve and follow its budget in compliance with the requirements of its funding source, g. Obtain, pay for and monitor its own telephone for making long distance calls and an answering machine for receiving confidential messages, h. Provide to Bridges copies of all required documentation, including but not limited to grant proposals, by -laws, personnel agreements, and personnel and program policies, i. Provide space in which supplies can be stored, records can be maintained, and where personnel and volunteers can work, b. Through and by its staff, assist per grant details, administer as specified in the grant, and compile necessary financial reports on a mutually agreed schedule, c. Provide accounting services as approved by the Bridges board of directors, including banking services regarding funds received, bookkeeping, payment of outstanding bills, preparation of payroll, and preparation of and submission of the appropriate forms for any employment taxes due, such taxes to be paid from each employers fund, d. Maintain and pay for a Post Office Box at which mail can be received, e. Take messages received at Bridges' phone number and forward as needed, f. Maintain a nonprofit mailing permit, which can be used for mailings, by Bridges, of materials which conform to Post Office regulations, g. Bridges agrees to provide access to miscellaneous services, including but not limited to: Borough sales tax exemption cards, use of the Bridges' credit card machine for a fee, use of the Bridges' State of Alaska gaming permit for raffles, and use of the photocopier, with the costs and/or fees associated with any such services actually used to be reimbursed by user, h. Bridges agrees to maintain and make available upon request all book, records, documents and other evidence pertaining to costs and expenses relating to this agreement to the extent and in such detail as will properly reflect all direct costs of labor, materials, equipment, supplies, services and other costs of whatever nature for which reimbursement is claimed or payment is made under these provisions. 5. This agreement shall be subject to all applicable provisions of State and Federal law and regulations related to the delivery and funding of nonprofit social services. SERVICE FEES AND EXPENSES 6. Miss Dee Dee's Class shall reimburse Bridges a 5% grant Administrative fee from each grant for which Bridges is responsible to be administrator, and agrees to reimburse any indirect or additional direct expenses incurred by Bridges and approved by its board, Pay the Bridges $25 yearly membership fee. CONFIDENTIALITY 7. Both parties agree that they shall be bound by and shalt abide by all applicable Federal or State statutes or regulations pertaining to the confidentiality of client records or information, including volunteers. The parties shall not use or disclose any information about a recipient of the services provided under this Agreement for any purpose not connected with the parties' contract responsibility, except with the written consent of such recipient, recipient's attorney, or the parent or guardian of recipient's who have a parent or guardian.
